Arthroscopic rotator cuff surgery

This procedure is to repair a torn rotator cuff which involves stitching the torn tendon back onto its attachment to the head of humerus.

Anchors (small pegs with thread) are placed on the head of the humerus and the surgeon will stick the tendon to pull it tight and to reattach it to the humerus using sliding knot technique.
The procedure needs more anchors in case of severe torn. Please look at the illustrations below to understand the surgical procedure.

Arthroscopy shows that the tendon is loosened and torn from the head of humerus.

Placing an anchor on the head of humerus

Two anchors are inserted

Looped suture is placed on tendon, anchor passes through the suture. Surgeons pull the suture to place the anchor on the tendon.

An instrument called suture grasper will be inserted into the tendon to help catch the anchors and place them on the tendon.

Surgeons will stitch the tendon to pull it tight and to reattach it to the humerus using sliding knot technique

Anchor is knotted to hold tight the tendons onto the head of humerus.

Rotator Cuff Repair Procedure

A Detailed Explanation and Surgical Technique

Repairing a torn rotator cuff involves stitching the torn tendon back onto its attachment to the head of the humerus. Small pegs with thread, known as anchors, are placed on the head of the humerus. The surgeon uses a sliding knot technique to pull the tendon tight and reattach it to the humerus.

Step-by-Step Surgical Procedure

  • The procedure requires more anchors in cases of severe tears.
  • Arthroscopy shows that the tendon is loosened and torn from the head of the humerus.
  • An anchor is placed on the head of the humerus.
  • Two anchors are inserted.
  • A looped suture is placed on the tendon, and the anchor passes through the suture.
  • The surgeon pulls the suture to place the anchor on the tendon.
  • A suture grasper instrument will be inserted into the tendon to help catch the anchors and place them on the tendon.
  • The surgeon stitches the tendon to pull it tight and reattach it to the humerus using the sliding knot technique.
  • The anchor is knotted to hold the tendons tightly onto the head of the humerus.

Irreparable large to massive rotator cuff tears

An irreparable rotator cuff tear refers to a complete tear of the rotator cuff tendons that is so severe or chronic that it is unlikely to heal with surgical repair. These tears are often massive, meaning they involve multiple tendons and/or are larger than 5 cm. Treatment options for irreparable rotator cuff tears typically focus on managing pain and restoring function, rather than fully repairing the tear.

President, Mori, produces their original methods for treating irreparable large to massive rotator cuff tears using autograft fascia lata patch graft procedures.
